HOW TO BECOME A MEMBER OF SRI LANKA LARYNGECTOMEE'S ASSOCIATION

1. Only Laryngectomees can become a member of Sri Lanka Laryngectomees' Association

2. He or She should be a Permanent Resident or Citizen of Sri Lanka

3. Take a printout of attached application form.

4. Handover the Duly completed form to our office at Department of ENT, National Hospital of Sri Lanka.
